Community Placement Plan (CPP) & Community Resource Development Plan (CRDP)
The Community Placement Plan (CPP) and Community Resource Development Plan (CRDP) were developed by the Department of Developmental Services (DDS) for regional centers as a means to enhance the capacity of the community service delivery system, reduce the reliance on the use of developmental center and other restrictive living environments, and ensure that individuals with intellectual and developmental disabilities live in the least restrictive setting, appropriate to their needs. Through the CPP and CRDP, DDS provides funding to regional centers for the development of a variety of community-based resources designed to meet the needs for services and supports for individuals living in the community, pursuant to Welfare and Institutions Code sections 4418.25 and 4679(a).

Service Provider for Adult Residential Facility for Persons with Special Health Care Needs (ARFPSHN)
Start-up Funds: $250,000
Home will be a 5-bed (non-ambulatory) Community Care Licensed Adult Residential Facility for Persons with Special Health Care Needs (ARFPSHN) that will provide 24-hour health care and intensive support services in a homelike setting for male and female medically fragile individuals with developmental disabilities needing community placement or deflection from Skilled Nursing Facilities.

Home will be a 4-bed (2 ambulatory, 2 non- ambulatory) Specialized Residential Facility (SRF) (Level 7), with Delayed Egress, to serve individuals with developmental disabilities, substance abuse and mental health service needs who require a structured, licensed setting while working towards transition to a less restrictive residential setting. These individuals will need support in substance abuse prevention, management, and treatment, anger and aggression management, medication management, and access to mental health services. The home will offer or arrange, substance abuse prevention and/or treatment services, comprehensive mental health counseling, trauma-focused therapies, social skills development, competency training, and crisis intervention services. Individuals to be served currently reside in community settings, rehabilitation programs, sober living homes, or are at risk of residing in locked facilities.
Specialized Residential Facility (SRF) for Adults with Forensic/Criminal Involvement
Start-up Funds: $200,000
Home will be a 4-bed (2 ambulatory, 2 non-ambulatory) Specialized Residential Facility (SRF) (Level 7) that will specialize in serving adult individuals with developmental disabilities and forensic/criminal involvement who require a structured, licensed setting while working towards transition to a less restrictive residential setting. Residents will likely be verbal, ambulatory and/or non-ambulatory, and are expected to have behavioral and/or co-occurring mental health support needs. Residents may have high incidents of verbal threats, and verbal and/or physical aggression. Individuals residing in the home may require forensic services such as court ordered diversion, competency training, recidivism prevention, mental health supports, and counseling, as well as support with enhancing coping and anger management skills.

Development of a training program for qualified providers that have demonstrated a commitment to providing quality residential services with behavioral supports (e.g., Level 6, SRF) that are interested in expanding their services to include EBSH and CCH homes.

Behavioral Day Service with Community Integration Training Component
Start-up Funds: $200,000
This licensed behavioral day service with community integration training component, to be located in the San Fernando Valley, will serve a minimum of 30 individuals with significant behavioral challenges and/or forensic involvement, as well as co-occurring mental health diagnoses. The program will serve individuals who require specialized services, including behavior management training, forensic services such as court ordered diversion, competency training, and recidivism prevention, psychological assessment, group psychotherapy, individual psychotherapy, medication management and monitoring, interpersonal skill development, and psychoeducational interventions. For those able to participate, the program will also include a component focused on the development of vocational skills and employment.

Substance Use Disorder Treatment Services Program
Start-up Funds: $50,000
This program will be an outpatient substance use disorder (SUD) treatment service licensed by the Department of Health Care Services (DHCS) for individuals with developmental disabilities and will include evidence-based practices that include relapse prevention, treatment, and recovery. Services to be delivered include, but are not limited to, intake and assessment, individual treatment sessions, group sessions, treatment planning, discharge planning, home visits, and consultation services. The program will also serve consumers who do not have primary substance abuse issues upon exiting a state developmental center or other restrictive setting, such as jail, but who may have a history suggestive of such a propensity when stressed.

Specialized Residential Facility (SRF) for Adults with Nursing Needs
Start-up Funds: $200,000
Home will be a 4-bed SRF for non-ambulatory individuals with nursing needs that do not require 24-hour care. Residents will need assistance with activities of daily living, safety, communication, socialization, and community integration. Residents may be cognitively and/or physically impaired (e.g. difficulty making socially acceptable decisions, non-ambulatory).
