Community Placement Plan/Community Resource Development Plan Request for Proposals (RFP)

North Los Angeles County Regional Center (NLACRC) is seeking proposal submissions for the following projects awarded by the Department of Developmental Services (DDS) as part of the Community Placement Plan/Community Resource Development Plan for Fiscal Year 2023-2024:

Enhanced Behavioral Supports Home (EBSH) for Children
Start-Up Funds: $250,000

Service provider for 4-bed (3 ambulatory, 1 non-ambulatory) Community Care Licensed EBSH with delayed egress for male/female children with intellectual disability and/or severe behavioral needs needing community placement or deflection from restrictive settings.

Enhanced Behavioral Supports Home (EBSH) for Adults (Male)
Start-Up Funds: $250,000

Service provider for 4-bed (2 ambulatory, 2 non-ambulatory) Community Care Licensed EBSH for adult male individuals with intellectual disability and/or severe behavioral needs needing community placement or deflection from Porterville Developmental Center, Canyon Springs, IMDs, or other restrictive setting.

Enhanced Behavioral Supports Home (EBSH) for Adults (Male) (Delayed Egress)
Start-Up Funds: $250,000

Service provider for 4-bed (2 ambulatory, 2 non-ambulatory) Community Care Licensed EBSH with Delayed Egress for adult male individuals with intellectual disability and/or severe behavioral needs needing community placement or deflection from PDC, Canyon Springs, IMDs, or other restrictive setting.

Adult Residential Facility (Level 4I) (Delayed Egress) for Adults with Substance Abuse and Mental Health Service Needs
Start-up Funds: $200,000

Service provider to develop a 4-bed (2 amb, 2 non-amb) Adult Residential Facility (ARF) Level 4I, with Delayed Egress, to serve individuals with developmental disabilities, substance abuse and mental health service needs who require a structured, licensed setting while working towards transition to a less restrictive residential setting.

Adult Residential Facility (Level 4I) for Adults with Forensic/Criminal Involvement
Start-up Funds: $200,000

Service provider to develop a 4-bed (2 amb, 2 non-amb) Adult Residential Facility (ARF) Level 4I that will specialize in serving adult individuals with developmental disabilities and forensic/criminal involvement who require a structured, licensed setting while working towards transition to a less restrictive residential setting.

Housing Development Organization (HDO) Acquisition and Rehabilitation of a Single Family Home (5-bed) to be Developed as an Adult Residential Facility for Persons with Special Health Care Needs (ARFPSHN)
Acquisition Funds: $300,000 Renovation Funds: $400,000

Acquisition and renovation by a non-profit Housing Development Organization (HDO) of a 5-bed (non-ambulatory) Community Care Licensed Adult Residential Facility for Persons with Special Health Care Needs (ARFPSHN) that will provide 24-hour health care and intensive support services in a homelike setting for male and female medically fragile individuals with developmental disabilities needing community placement or deflection from Skilled Nursing Facilities.

Housing Development Organization (HDO) Acquisition and Rehabilitation of a Single Family Home (4-bed) to be Developed as an Enhanced Behavioral Supports Home (EBSH) for Adults
Acquisition Funds: $300,000 Renovation Funds: $400,000

Acquisition and renovation by a non-profit Housing Development Organization (HDO) of a 4-bed (2 amb, 2 non-amb) Community Care Licensed Enhanced Behavioral Supports Home for male and female individuals with developmental disabilities and intensive behavioral needs requiring community placement or deflection from PDC, Canyon Springs, STAR, and/or IMDs.
