SLS Orientation & Information Packet

Supported Living Services (SLS) provides you support so that you can live in your own apartment and be part of your community. At the SLS orientation, you will learn what SLS is so that you can decide if this is an option that works for you based on your individual circumstances. You will learn the basic requirements to participate in the SLS program, things you need to consider in planning for SLS, how to get started, and most of all, that in SLS, you are the one in control. You pick your provider, you hire the staff and you decide on your plan, and you get as much or as little assistance as you feel you need.

Orientation is held from 3:00 pm – 4:00 pm.

Due to the COVID-19 health crisis, our SLS orientations are currently being conducted virtually until further notice, and you can attend by computer or phone. We are not currently conducting these orientations in our offices.

Consumers age 18 and over who are interested in supported living should let your Service Coordinator know if you are interested in attending an SLS orientation. You will be provided the virtual orientation log-in or dial-in information closer to the orientation date.
