Ang Community Placement Plan (CPP) at Community Resource Development Plan (CRDP) ay binuo ng Department of Developmental Services (DDS) para sa mga sentrong pangrehiyon bilang isang paraan upang mapahusay ang kapasidad ng sistema ng paghahatid ng serbisyo sa komunidad, bawasan ang pag-asa sa paggamit ng developmental center at iba pang mahigpit na kapaligiran sa pamumuhay, at tiyakin na ang mga indibidwal na may mga kapansanan sa intelektwal at pag-unlad ay naninirahan sa pinakamababang paghihigpit na kapaligiran, na naaangkop sa kanilang mga pangangailangan. Sa pamamagitan ng CPP at CRDP, ang DDS ay nagbibigay ng pagpopondo sa mga sentrong pangrehiyon para sa pagpapaunlad ng iba't ibang mapagkukunang nakabatay sa komunidad na idinisenyo upang matugunan ang mga pangangailangan para sa mga serbisyo at suporta para sa mga indibidwal na naninirahan sa komunidad, alinsunod sa mga seksyon ng Welfare and Institutions Code 4418.25 at 4679( a).

Start-up Funds: $250,000
Service provider para sa 4-bed (2 ambulatory, 2 non-ambulatory) Community Care Licensed EBSH para sa mga indibidwal na nasa hustong gulang na lalaki na may kapansanan sa intelektwal at/o malubhang pangangailangan sa pag-uugali na nangangailangan ng pagkakalagay o pagpapalihis ng komunidad mula sa Porterville Developmental Center, Canyon Springs, IMDs, o iba pang mahigpit na setting.

Service Provider for Enhanced Behavioral Supports Home (EBSH) for Adults (Co-ed)
Start-up Funds: $300,000
Service provider for 4-bed (2 ambulatory, 2 non-ambulatory) Community Care Licensed EBSH for adult individuals with intellectual disability and/or severe behavioral needs needing community placement or deflection from Porterville Developmental Center, Canyon Springs, IMDs, or other restrictive setting.

Service provider for a 4-bed (2 ambulatory, 2 non- ambulatory) Adult Residential Facility (ARF) Level 7, with Delayed Egress (formerly known as Specialized Residential Facility – SRF) to serve individuals with developmental disabilities, substance abuse and mental health service needs who require a structured, licensed setting while working towards transition to a less restrictive residential setting.

Service Provider for Adaptive Skills Training for Children
Start-up Funds: $30,000
This service will provide ongoing Socialization and Safety Training for youth and young adults with intellectual disability and/or severe behavioral needs residing in an Enhanced Behavioral Supports Home (EBSH), licensed residential facility, or who are at risk of placement in a licensed setting. Services will be provided outside of school hours.

Emergency Short-Term Shelter Services
Start-up Funds $60,000
Program to provide short-term (approximately 3–5 days) emergency shelter services to support clients who must be displaced during disasters at two (2) existing Community Care Licensed facilities—one in the San Fernando Valley, and the other in the Antelope Valley.

Development of new or expansion of existing Housing Access Services businesses resulting in the establishment of local, regional Housing Access Services that will utilize the On My Own Roommate Matching app. Joint project with Tri-Counties Regional Center.
