What consents do I need to submit with my human interest story?

Each person appearing in, who is heard in, or who is talked about in, a
human interest story video, photo, or recording (as applicable) must complete
and sign the “Authorization, Consent, and Release for Use of Name, Voice and
Likeness.”
Each person who took or made the picture, video, or recording must complete
and sign the “Photography/Video Release.”
