PROJECT OVERVIEW
The North Los Angeles County Regional Center (NLACRC) is releasing this Request for Vendorization (RFV) to seek qualified individuals/agencies to provide Coordinated Family Support (CFS) to participants and their families within the NLACRC catchment area.
In June 2022, the State’s Budget Trailer Bill for developmental services, SB 188 (Chapter 49, Statutes of 2022), added Welfare and Institutions Code section 4688.06, establishing the Coordinated Family Support Services Pilot Program. This section recognizes the right of adults with disabilities to reside in the family home and that adults with developmental disabilities, and their families, may need CFS services that are tailored to the unique needs of the consumer and are respectful of the language, ethnicity, and culture of the family home. The CFS Services Pilot Program focuses on improving equitable access to services and supports and reducing ethnic and racial disparities in the purchase of services. IMINUMUNGKAHING MGA MODELO NG SERBISYO
A regional center shall classify a vendor as a CFS provider when the vendor coordinates and provides items identified below for adults who reside in the family home. CFS shall be tailored to the unique needs of the consumer and their family and provided in a manner that respects their language and culture. It shall be primarily provided in a person’s home. CFS may include, but is not limited to:
- Identifying and providing supports necessary to successfully reside in the family home.
- Providing assistance and training for the consumer and their family in navigating comprehensive services and supports that are tailored to meet their unique needs, including creating pathways to overcome barriers to accessing generic and other resources.
- Providing additional information or resources on the consumer’s diagnosis and identified supports.
- Coordinating consistency in training across providers specific to the needs of the consumer and their family.
- Assisting with scheduling of service delivery including medical and other appointments.
- Identifying transportation options or services.
- Identifying backup providers/supports and providing those backup supports when the plan fails.
- Providing futures planning for the consumer, including those living with aging caregivers.
- Providing training to the consumer which maximizes their independence.
CFS shall not replace or duplicate any regional center service coordination, generic service or other regional center funded service that the consumer and their family are receiving. CFS may not be provided by an individual who resides in the same home as the consumer. At minimum, the need shall be assessed annually, with progress being reported quarterly.
The Department created a standard referral and assessment tool for service coordinators and CFS providers to use when referring and assessing consumers for CFS (Enclosure C). Step-by-step instructions are provided on the form. The Department also created a standard tool for CFS providers to report to regional centers on the consumer’s progress (Enclosure D).

This service will be provided to adults diagnosed with a developmental disability, found eligible for regional center services who also are in need of supports while residing in their family home.
LOCATION & LANGUAGE SPECIFIC
Coordinated Family Support services are being requested to be provided in any and/or all three valleys located within the NLACRC catchment: San Fernando Valley, Antelope Valley and Santa Clarita Valley. In addition, services are being requested in at least 1-3 other languages, besides English, listed below. See Proposal Title page for details.

RATE NG REIMBURSEMENT/RATIO
The Department has established rates for CFS. NLACRC is able to reimburse CFS services as a rate of $60.02/hr. There is no start-up funding available for this vendoriztion. All Coordinated Family Support services will be delivered at a 1:1 provider to participant ratio.
Each provider that meets all monthly reporting requirements will be eligible to receive CFS Pilot Implementation Incentive Payments. These payments will be calculated at 11.1% of the total dollar amount billed for CFS assessments and services for the prior month. Applicants responding to the RFV shall bear all costs associated with the development and submission of a proposal.
MINIMUM KUALIFIKASYON PARA SA MGA APPLICANT
Minimum qualifications for CFS staff include an Associates-level degree in a human services field of study or 3 years of experience in the developmental disabilities service delivery system, communicating in the primary language of the consumer and their family, and knowledge of the regional center system.
Minimum qualifications for CFS supervisors include a Bachelors-level degree in a human services field of study or an Associates-level degree in a human services field of study and at least 3 years of experience in the developmental disabilities service delivery system.
Within the first 30 days of working with a consumer and their family, CFS providers shall provide training to their staff and supervisors on the service delivery systems including, but not limited to, behavioral health services and local Area Agencies on Aging. Exceptions to the minimum qualifications, along with the justification, shall be submitted to the regional center for the Department of Developmental Services’ determination.

Both non-profit and proprietary organizations are eligible to apply. Employees of Regional Centers are not eligible to apply. Applicants must disclose any potential conflicts of interest per Title 17 Section 54500. Applicants, including members of the governing board, must be in active status in regard to all services vendored with any regional center and be financially solvent. Applicants that have been sanctioned in the last 12 months will not be eligible for vendorization. CFS may not be provided by an individual who resides in the same home as the consumer.

RESERVATION NG KARAPATAN
NLACRC reserves the right to request or negotiate changes in a proposal, to accept all or part of a proposal, or to reject any or all proposals. NLACRC may, at its sole and absolute discretion, select no provider for these services if, in its determination, no applicant is sufficiently responsive to the need. NLACRC reserves the right to withdraw this Request for Vendorization (RFV) and/or any item within the RFV at any time without notice. NLACRC reserves the right to disqualify any proposal which does not adhere to the RFV guidelines. This RFV is being offered at the discretion of NLACRC. It does not commit the regional center to award any grant. Please note that applicants must be in active status with NLACRC and other Regional Centers and may be disqualified for any of the following: receipt of Correction Action Plan (CAP), Sanction or Immediate Danger findings, failure to disclose any history of deficiencies or confirmed reports of consumer abuse, previous failure to perform, or unwillingness to comply with Title 17 and NLACRC best practices.

Ang aplikante ay kinakailangang magsumite ng elektronikong kopya sa a PDF format. PAGSASABALA NG MGA MUNGKAHI
