The meeting offers an opportunity for individuals and provide valuable feedback and comments.

The following items will be presented:

1. Child Family Survey 2021-22 – Completed by families of a child (ages 3-16 years old) who lives in the family home.

2. Adult Family Survey 2021-22– Completed by families of an adult (age 18 and over) who lives in the family home.

3. Family Guardian Survey 2021-22– Completed by families or conservators of individuals (age 18 and over) who live in a community placement.

4. Adult In-Person Survey 2017-18 – In-Person Survey (IPS) is a face-to-face meeting conducted with a person who is receiving services.

5. Adult In-Person Survey 2020-21 – In-Person Survey (IPS) is a face-to-face meeting conducted with a person who is receiving services.
