Informe de incidente especial (SIR)

DDS (Department of Developmental Services) SIR Reporting Requirements:-Effective Monday, April 3, 2023, DDS is ENDING SPECIAL INCIDENT REPORTING FOR positive COVID-19 and MONKEY-POX diagnosis to Regional Centers.  This also includes reporting on COVID-19 vaccination status.  Vendors/providers & LTHC facilities are no longer required to submit SIR’s to the Regional Center for positive COVID-19 or Monkey Pox diagnosis. 

CCLD (Community Care Licensing Division Reporting requirements): 

-As of March 1, 2023, CCL Licensees are no longer required to report on individual cases of COVID-19 to Community Care Licensing Division. Some COVID-19 reporting remains in place

-Adult & Senior Care Licensees are required to report suspected or confirmed outbreaks to their local health department. 

-Child Care Center Licensees, are required to report epidemic outbreaks to their local regional office and to their local health department. 

-Family Child Care Home Licensees are required to report a communicable disease outbreak when determined by their local health department to their local regional office. 

-Children’s Residential Licensees and providers are still required to report epidemic outbreaks to their local regional offices and to their local health department.  

The California Department of Developmental Services (DDS) and regional centers monitor the occurrence of adverse events, captured through Special Incident Reports (SIRs), in an effort to identify trends and develop strategies to prevent and mitigate risks to consumer health and safety. 
 
As required by Section 54327 of Title 17 of the California Code of Regulations, vendors and long-term health care facilities must report occurrences of suspected abuse, suspected neglect, injury requiring medical attention, unplanned hospitalization, and missing persons, if they occur when a consumer is receiving services funded by a regional center (under vendored care). 
 
In addition, any occurrence of consumer mortality or a consumer being the victim of a crime must be reported whether or not it occurred while the consumer was under vendored care. (from DDS Semi-Annual Report on Mortality, July- Dec 2009) 

New procedures for reporting elder & dependent abuse to Adult Protective Services 

Please be advised that Adult Protective Services has eliminated its Centralized fax number, and the procedure for reporting suspected Elder and Dependent Adult Abuse and Neglect to Adult Protective Services (APS) has changed. 
 
You may report an incident of suspected Elder and Dependent Adult Abuse and Neglect to APS, by choosing one of the two options listed below: 
 
1. Verbally report the incident by calling the: 
a) 24-hour Elder Abuse Reporting Hotline at (877) 477-3646 or 
b) APS Mandated Reporter Hotline at (888) 202-4248. 
 
2. Complete the SOC 341 form and mail it to:

OR 
 
1. Submit an electronic report at https://wdacs.lacounty.gov/services/older-dependent-adult-services/adult-protective-services-aps/. Please note: if you submit a report electronically, you do not need to verbally report the incident. Additionally, currently there is no option to print the electronic report, and once submitted, you will not be provided with a confirmation number. APS is currently working on adding a print option and other improvements. 
 
Follow up Instructions: 
After submitting the written report, please call APS after a couple days at (213) 351-5401 to inquire about the status of the case.
